When it was established, the Maccabi Health Fund numbered ten doctors and several dozen members. The founding fathers of the fund include: Dr. Felix Tilehber, Dr. Ernest Freudenthal, Peretz Bernstein, Uri Nadav, Dr. Avigdor Koren, George Flesh, Dr. Eliyahu Sedger, Hanania Berger, Dr. Moshe Hernick, Dr. Eliezer Ludwig Lerfreund, Dr. Robert Atlas, Felix Sheiniak, Arthur Rosenberg, Fritz Levinson, Dr. Herman Leleber. Most of them were doctors and some were managers and financier.

Maccabi doctors from Central Europe brought with them liberal working methods, centered on the member's free choice of doctor and institute. The method of activity was based on a limited staff and on self-employed doctors who were remunerated according to their activity. Maccabi's medical treatment is based on principles, the main ones being the provision of public medicine at the level of private medicine. This is a direct approach to advanced health services of its kind through senior physicians.

Since 1995, Maccabi has been operating under the National Health Insurance Law. The rate of insurance premiums is determined by law and collected by the National Insurance Institute. Maccabi institutions are staffed by first-rate public figures, who are elected in elections that take place every four years. All Maccabi members have the right to vote and be elected.
